Predictors of interventions in a sample of patients with frequent emergency department visits.
Frequent emergency department (ED) use has been associated with emergency physicians (EPs) often finding patients with serious problems. However, few have examined the predictors of intervention within the ED. The objective was to determine if patient, physician, and practice characteristics are associated with rate of intervention at the ED. This was a retrospective medical record review of ED visits by adult patients, with five or more ED visits in the previous year, at a single urban teaching hospital. Patient and physician characteristics were assessed by chi2 analysis. Most patients had one to four visits in the previous year and were female (74%), nonwhite (82%), and aged 45-64 (49%). Although approximately half of the patients had selected a primary care physician (PCP) at their previous visit, only 2% had a PCP with whom they had a positive provider-patient relationship. Half of the visits were to nonhospital locations such as urgent care centers. Over half the patients had at least one chronic medical condition. The physician was seen for a mean of 11.7 (SD 10.7) minutes. Patients with a chronic medical condition were more likely to have a visit to a nonhospital location (P =.04). The patient was seen for a mean of 28.7 (SD 16.9) minutes. Only 40% of the patients had a positive provider-patient relationship, which was associated with a medical condition, language spoken at home, and having a primary care physician (P 827ec27edc
