All three oral antivirals are designated Pregnancy Category B. (Category B: Animal reproduction studies have failed to demonstrate a risk to the fetus and there are no adequate and well-controlled studies in pregnant women.) However, a large population-based, historical cohort study of 837,795 live-born infants in Denmark found no association between first trimester exposure to valacyclovir or acyclovir and major birth defects.229 Famciclovir was also included in this study, but exposure to the drug was uncommon and the evidence insufficient to support its safety during pregnancy. In spite of their similar safety and efficacy profiles, there may be advantages to choosing one oral antiviral agent over the other in select cases.
